FMEA (Failure Mode and Effects Analysis) is specific to manufacturing processes, is an analytical methodology used to ensure that potential problems have been considered and addressed throughout the product and process development process. Goes through risk ranking and calculating risks numerically, leading to actions to minimise.

Who will benefit?

This will benefit those individuals in product or service sectors involved in designing, producing, running, managing or maintaining products/processes. It applies to all levels of an organisation as all can input to an FMEA.

Workshop Objectives

Despite best efforts, products or processes are often created and put into operation yet, somehow, don’t quite deliver what they are required to do. Issues arise that no one has thought about and this is often due to lack of a detailed approach to consider upfront ‘what potentially could go wrong’.

Failure Mode Effects Analysis (FMEA), often associated with manufacturing but also relevant to service providers, is an effective and simple analytical methodology used to ensure that potential benefits and issues have been considered and addressed throughout the product and process development process in order to minimise risk to customers, organisational employees and other stakeholders. FMEA is also applied retrospectively to the product/process in order to bring about benefits.

By identifying potential failure modes, looking at existing controls and risk ranking each using a simple but comprehensive numerical approach, necessary additional controls can be identified and implemented to eliminate or reduce the risk of potential failure modes impacting on the product/process and reducing the risk of e.g. quality issues, supply issues, H&S issues etc. ie. ‘less things to potentially go wrong’.

Focussing on Process FMEA (PFMEA), the outline aims of the workshop, through application of both theory and practice are to identify and understand:

● What is an FMEA and where used?’;

● Benefits and types of FMEA;

● FMEA Terminology;

● Risk Rating System and Application;

● How to develop and maintain your PFMEA;

● Examples of PFMEA;

● Creating your own PFMEA and identifying any actions necessary to reduce risks.

*NB - Please bring along your laptop - you will be sent a process capability and FMEA spreadsheet in advance to use on the day. (If you require access to a laptop please let us know in advance.)
